BMC-91X, BMC-34, BMC-84: FMCSA insurance forms explained

When you look up a carrier, its insurance shows up as FMCSA "forms". Here's what the common ones mean.

BMC-91 and BMC-91X — liability (BIPD)

Filed by the insurer to certify bodily injury and property damage liability coverage. "91X" is used when the carrier's coverage is provided by multiple or excess layers. This is the coverage most brokers check first.

BMC-34 — cargo

Certifies cargo insurance. Federal cargo filings are mainly required for household goods carriers; many shippers require cargo coverage by contract anyway.

BMC-84 and BMC-85 — broker and forwarder security

A BMC-84 is a surety bond and a BMC-85 is a trust fund agreement, filed for property brokers and freight forwarders (currently $75,000).

Cancellations

Insurers must notify FMCSA when they cancel. A cancellation with a future effective date is an early warning that coverage is about to end.
